UW Medical Center – Northwest has an outstanding opportunity for a Case Management Assistant in the inpatient setting. The Case Management Assistant will be responsible for working closely with the Social Worker, Nurse Care Coordinator, Physician, and the patient to arrange follow-up appointments post-discharge and assist with coordination of the discharge planning process for patients.
SCHEDULE:
Full time, day shift, 8 hours per shift, 40 hours per week.
RESPONSIBILITIES:
Arrange hospital discharge follow up appointments and referrals
Assist in arranging home services, such as home health, wound vac, or in-home caregivers, based on information obtained from clinical staff
Assist in arranging practical needs and emergency assistance, such as transportation, parking, or local lodging. Demonstrate problem solving skills in working with resources outside the medical center
Process and send clinical updates to payors
Research and update community resource information as needed
Participate in the training of Social Work Students and new employees
Document interventions in electronic medical record
REQUIREMENTS:
Bachelor of Social Work or a Bachelor’s Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent.
OR
an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.
Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
DESIRED:
Ideal candidates have experience working in a medical setting, demonstrate strong multi-tasking and organizational skills, oral and written communication, and computer proficiency. Must work well with a variety of persons and teams and possess excellent customer service skills.
