As a Case Manager with us, you will have the opportunity to be involved in the coordination of care and services for patients admitted to Saint Joseph Hospital / SCL Health who are identified as needing assistance or meeting Care Management criteria.

Your day.

As a Social Work Case Manager you need to know how to:

Coordinate the care and services for patients identified as needing assistance or meeting care management criteria.

Assess the patient’s prior level of functioning, access to and/or use of community resources and available support systems.

Identify appropriate admission and continued stay issues.

Attend, facilitate, and participate in rounds and case conferences.

Identify needs, facilitate, or provide education to physicians and ancillary departments/nursing units regarding case management, discharge planning process, and roles.

Participate in multidisciplinary groups and development of guidelines.

Your experience.

We hire people, not resumes. But we also expect excellence, which is why we require:

Graduate of an accredited school of social work, sociology, or related field

Minimum of one (1) year of clinical experience

Preferred:

At least two (2) years of case management experience in a hospital setting
