Posting Summary
Rutgers, The State University of New Jersey is seeking a Patient Navigator for the Department of Medicine-Infectious Disease within the New Jersey Medical School. Under the direction, the Patient Navigator will administer HIV Counseling, Testing and Referral Services in various settings on the Newark campus recruiting and maintaining a caseload of clients for LIGHT Program. The Patient Navigator will oversee resources including but not limited to: education, engagement, enrollment, patient navigation and adherence counseling program at Rutgers, New Jersey Medical School. The Patient Navigator will also actively participate in community engagement as it relates to HIV Testing and promotion of care and treatment.
Among the key duties of this position are the following:
Promotes goals and objectives, internally and externally for HIV and Hepatitis C Counseling, Testing and Referral Services (CTRS) for the Rutgers Newark campus and Essex County.
Follow guidelines and procedures set forth by the New Jersey State Department of Health and Human Services (NJDHHS) around HIV and Hepatitis C CTRS.
Conducts quality controls daily and weekly (e.g. room temperature, freezer temperature, run controls, run performances, in -house and RWJ testing logs).
Coordinates and performs HIV Counseling, Testing, and Referral within Rutgers and community.
Engages and links newly identified HIV-infected patients into care and treatment as per Standing Operating Procedure (SOP).
Provides referrals to Partner Notification Assistance Program as necessary as per SOP

Qualifications
Minimum Education and Experience
Bachelor’s Degree in Social Work, Counseling, Psychology or relevant field, and required two (2) years’ experience as an HIV tester/counselor.
Certifications/Licenses
Required Knowledge, Skills, and Abilities
Computer literacy with competence in Microsoft Office components.
Excellent interpersonal skills and ability to work as a member of a team.
Must possess excellent written and verbal communication skills; strong organizational skills; detail oriented; able to function independently and work flexible hours; friendly, outgoing, sociable; open-minded & non-judgmental; caring, empathic, compassionate; committed, dedicated, persistent; creative & resourceful.
Preferred Qualifications
New Jersey Department of Health and Human Services (NJDOHHS) training and certification and/or obtained/recertified within 90 days of date of hire.
NJDOH HIV counselor certification.
Knowledge of Newark community.
Bilingual skills (English/Spanish).
