POSITION SUMMARY:
The Population Health Coach(PHC) is responsible for a panel of patients and, in collaboration with other members of an integrated care team, helps the patients meet their preventative, chronic and acute care needs. Health coaches engage patients and encourage them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices and adapt lifelong healthy behaviors. The PHC will establish contact with patients with transitional and chronic care needs to ensure their understanding, agreement and participation in wellness. Demonstrate superior organizational & interpersonal skills; will be detail oriented; will have proven ability to work collaboratively, and be able to multi-task. Facilitate the patient’s process of gaining the knowledge, skills, tools and confidence to become active participants in their care. Will play a key role in building a culture of health and safety in support of the organizations vision, mission, and values.
EDUCATION:
Associates degree, Bachelor’s degree preferred and background in health related field
EXPERIENCE:
Experience: 1-2 years of work experience in a Healthcare organization or hospital system preferred. Knowledge of health promotion, nutrition, and counseling preferred. Strong interpersonal skills; demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.
SPECIAL SKILLS:
o Demonstrate an ability to deliver effective telephonic coaching in a manner that promotes behavior change for our populationo Obtains and reviews pertinent medical history, current diagnosis, and pharmaceutical data for Population Health identified patients.o Provides telephonic coaching and information to Population Health patients, managing various chronic health conditions.o Communicates with Population Care Nurse, physicians, and other health professionals to provide care coordination that facilitates the efficient use of health care resources.o Effectively coaches, evaluates and graduates patients from care in conjunction with the care management team.o Assists in the transition of care across the continuum. Uses evidence-based coaching techniques to help members achieve healthy lifestyle behavior challenges, align lifestyle behaviors with their personal wellness goals, and design a realistic action plan in support of a healthier lifestyle.o Provides exceptional service and/or care to all patients, customers and team members. Engages, educates, and promotes/influences members in decisions related to achieving and maintaining optimal health status. Delivers a high quality member coaching experience resulting in positive behavior change.o Participate effectively as a team member to share ideas across delivery networks and ensure that local goals and objectives are consistent with system-wide wellness, population health management, and human capitol goals.o Responsible for coordinating wellness and prevention programs based on the strategic population health management goals for our defined client populations to include but not limited to; educational seminars, health screenings, behavior change programs, wellness challenges, access to available resources, and special events
