Job Description : CHC Care Coordinator Job Description Position: CARE COORDINATOR Reports to: ADMINISTRATOR Hours: 40hr/week Wage: $22/hr Summary of Responsibilities: Schedules and supervises home care agency staff. Assumes responsibility for the delivery of quality home care services.Specific duties include: 1. Schedules and monitors the work of all agency direct care personnel and contractors.2. Oversees Office Phone and After Hours Phone, have working knowledge of emergency procedures and provide direction3. Must have active caregiver license and be ready and able to fill shifts4. Oversees Caregiver Recruitment such as maintaining job posts, doing community outreach, hosting job fairs, conducting interviews, etc5. Performs annual evaluations of all direct care staff, including on-site visits for observation and evaluation of care delivery.6. Communicates with clients, their representatives and with caregivers to assure appropriate client care.7. Promotes effective home care services in compliance with agency philosophy, policies and procedures, and in accordance with State licensing laws and regulations.8. Assures compliance with the home care plan of care by agency direct care staff.9. Maintains current home care knowledge through attendance at in-service and continuing education programs.10. Assists with the evaluation, development and revision of agency policies and procedures.11. Assists in the implementation and oversight of the agency quality assurance plan.12. Coordinates caregiver attendance at trainings.13. Performs other duties as assigned by administratorQualifications: 1. Work experience in the delivery of social services, health care or a related field.2. Work experience with a complex scheduling system.3. Must demonstrate good communication, organizations and management skills.4. Understanding of the psychology of caregiving and its importance to vulnerable and/or disabled persons.5. Knowledgeable of Washington State rules and regulations for licensure of a home care agency.6. Ability to perform and prioritize multiple functions and tasks and deal with multiple changes.7. Ability to self-lead and self-direct, confident to work independently8.
