Job Description : GRIEF CARE COORDINATORTITLE: Grief Care CoordinatorREPORTS TO: Grief Support ManagerQUALIFICATIONS1. Bachelor of Science or Bachelor of Arts in the fields of education, communication or social service practices required. Experience meeting with individuals, families, and/or caregivers to address anticipatory grief or bereavement needs related to dying and death required.2. Ability to work as a team member.3. Knowledge of and commitment to Hospice philosophy.4. Understanding of grief response and experience with supporting individuals and/or in groups regarding hospice death or non-hospice death (community).5. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.6. Successful criminal records check completed by the BCI at time of hire and as required by agency. Successful Motor Vehicle Record check completed by Ohio Bureau of Motor Vehicles at time of hire and as required by agency.SUMMARY OF JOB RESPONSIBILITIES Responsible to provide individual/group grief care by meeting in person or via telehealth to address the anticipatory grief or bereavement needs of hospice family members (hospice death) and community family members (non-hospice death).RESPONSIBILITIES AND DUTIES1. Provide child, teen and adult grief care individually/in groups via telehealth or meeting face to face at the Anita M. Turner Center, area schools/facilities, homes of hospice families, or Pickering House to ensure timely and appropriate grief care is provided.2. Work in conjunction with Case Managers, Pickering House staff and Social Workers. Seek guidance from other members of the interdisciplinary team as needed.3. Responsible for documenting grief care on assessments, plans of care, contact notes, and team summaries.4. Collaborate with Pickering House staff regarding family members who are having trouble communicating or processing their grief reactions related to a patient’s dying.GRIEF CARE COORDINATOR5. Assist in maintaining the database for the grief care caseload.6. Assist with providing grief care information to callers from the community.7. Participate in grief care events as needed..8. Provide grief care to Hospice personnel coping with work related grief through one-to-one support and referral to community resources.9. Participate in Hospice activities such as in-service education, Hospice personnel meetings, and relevant committees.10. Attend and actively participate in interdisciplinary team meetings.11. Advance professional knowledge and skills.12. Accept and perform other related duties and responsibilities as required.13. Document services, referrals, etc. for the survivor’s clinical record.14. Promote an atmosphere conducive to successful interrelationships of staff.15. Comply with agency policies and procedures.16. Adhere to privacy practices as described in Notice of Privacy Practices.17.
