Planned Parenthood Arizona services include an emphasis on family planning and reproductive health care, including the provision of birth control and comprehensive contraceptive counseling, testing and treatment of sexually transmitted infections, pregnancy testing, counseling and referral, HIV testing, annual exams, abortion care and more. In addition, PPAZ focuses on providing medically accurate sexuality education and training services throughout the state.
The Patient Navigator is a community health professional that focuses on the abortion patient’s needs to access care on both an individual basis, and at a systems level, while relieving as much of the logistical and practical burdens on the patient as possible. The patient navigator helps guide the patient through the abortion care system and works to overcome obstacles faced by the patient receiving the care and services they require, while working to improve the systems and processes that shape access at the clinical and ecosystem levels.
Patient navigator will be mainly responsible for scheduling appointments and/or referrals to other clinical care sites, coordinating health care activities of the patient, conducting financial intake and/or helping to secure funding for their abortion if necessary and organizing logistical and practical support if necessary. The patient navigator will help patients identify needs and connect patients to appropriate resources and supports. The navigator may be involved and coordinate with outside entities as well. The patient may have needs to arrange legal counsel, transportation and travel logistics, dependent care or anything else necessary to help them obtain their abortion that the patient navigator will assist with. The patient navigator collaborates with diverse organizations and personnel to ensure the needs of the patient are met. The ultimate goal of the patient navigator is to relieve as much of the logistical and practical burdens from the patient as possible, such that the path to obtaining high-quality, patient-centered abortion care is as clear and smooth as possible. As such, the role is also well positioned to identify, address, and improve the systems, structures and processes that create barriers to care at the level of the call center, health center, and ecosystem.
Patient Care Services & Advocacy:Works with all staff to ensure appropriate access to services by current and potential patients, and seeks always to maximize our mission;Organizes schedules and manages appointments for patients to ensure they receive services in a timely manner and at a location most convenient for them based on the patient’s clinical care site preferences.Facilitates pre and post-abortion visits and ensures continuity of care and linkage to community resources.Facilitates communication between the patient, support person(s) and/or organization(s), and healthcare providers to ensure patient satisfaction and quality of care.Informs the patient on financial aspects of care and links the patient to financial resources as needed.Works with the patient to identify logistical needs to access care and links the patient to appropriate resources as needed.Works with the patient to identify other support services as needed and provides appropriate linkages.Accommodates interpretive language services when needed.Addresses patient and employee concerns, finding solutions and resolutions to issues or complaints.;Responsible for quality and compliance, seeking always to provide customer-centered quality care in a safe environment for both patients and staff.Performs outreach to the community.Embeds equity in all levels of work; evaluates efficacy from a lens of racial equity and targets effective interventions to address gaps; etc.
Coordination with other staff:Works with external stakeholders and organizations to build and strengthen relationships, systems, and processes that support access to abortion care.Works with external vendors to facilitate access to key services, including but not limited to, transportation and lodging.Works closely with the Planned Parenthood Federation, affiliates and call center staff, including but not limited to the Director of Healthcare Operations, other Health Center Managers, Senior Director of Equity, Diversity, and Inclusion, and others to create efficiencies and improve workflows for increased abortion access and improved patient experience.
Administrative functions:Develops, manages, and advances program goals and objectives for new and/or improved processes and systems that increase access to equitable abortion care.Collects, tracks, and reports identified success measures to include but not limited to: patient satisfaction survey data, successful access data, EDI KPI’s and program cost.Identifies and addresses existing gaps and opportunities within existing patient navigation processes, including screening processes, to ensure highest-need patients are being identified, routed, and connected to navigational supportIdentifies process improvement opportunities and presents solutions to the Director of Health Care Operations; generates monthly patient survey data and monitors patient feedback and customer service metrics.
FinancialAdheres to agency policy with regard to patient payment structures including insurance, payment at time of service and payment arrangements.
Work EnvironmentThe work environment may vary depending on whether the incumbent works in an office or remote.This position usually involves daytime work, but sometimes night and weekend hours may be expected.Patient navigators may work with anyone seeking abortion care that needs assistance.The patient navigator works closely with the health care providers, clinic staff, local and national abortion funds and other community organizations and volunteers.

EDUCATION and/or EXPERIENCE:
Bachelor’s Degree.
Experience working in a clinical setting is preferred. Background in social work preferred. Bilingual (English-Spanish) preferred.
Strong computer skills to include Microsoft Office software and use of the internet.
The ability to initiate and follow through on new projects is required. Must be able to make recommendations to effectively resolve problems or issues, by using judgment that is consistent with standards, practices, policies, procedures, regulations or government law. The ability to work effectively and confidentially with executives, managers and employees. Must have strong time and stress management skills, attention to detail and the ability to work in a multi-tasking environment.
