Job Description : The Scheduler/Case Manager works collaboratively with clients, families and Care Specialists to identify, coordinate, and provide individualized services to clients. They supervise the delivery of services to ensure that services provided are in accordance with CDPHE regulations, and to ensure client and family satisfaction. They collaborate closely with other members of the Client Services department, as well as other office staff members.Essential Duties: Displays professionalism, kindness, integrity, collaboration, and a can-do attitude in all interactions with employees, clients and family membersDemonstrates an understanding of CDPHE regulations and Agency policies and is able to effectively communicate these to employees, clients and families as necessaryDemonstrates a thorough knowledge of the needs and concerns of the clients to whom they are assigned as a Case Manager, and a general knowledge of the needs and concerns of all other Agency clientsWorks closely with other Schedulers, Case Managers, Client Services Support Specialists, Intake Specialists, and Consultants on scheduling and other client-related issuesSupports staffing decisions made by colleaguesDemonstrates careful attention to detail especially when schedulingPerforms regular supervisory visits for clients and Care SpecialistsIntroduces new clients and new Care SpecialistsMaintains accurate and complete client records, ensuring Care Plans are up to dateCompletes and performs regular performance evaluations for Care SpecialistsCoaches and encourages Care Specialists toward the goal of professional excellenceCollaborates with the Manager, and the Human Resources department to resolve employee discipline issuesEnsures incidents and complaints are promptly investigated, resolved, and accurately documentedProvides on-call support in the evening, on weekends, and on occasional holidays as assignedProvides direct client support on an as needed basis, as directedTracks data regarding missed/cancelled shifts, hospitalizations, ER visits, and other details as requestedDemonstrates proficiency using Microsoft Office and is receptive to the use of technology to improve operations and overcome problemsResponds to new business calls as neededCollaborates with other Agencies involved in the clients’ carePerforms miscellaneous clerical tasks associated with client services and schedulingDemonstrates an understanding of HIPAA practices and is able to communicate these to clients, families and Care SpecialistsComplies with all State Guidelines of ServiceOther Duties: Verifies the need for holiday care with clients and families and communicates holiday schedules with Care SpecialistsParticipates in Annual Training and other office eventsOther duties, as assignedJob Qualifications: Must be at least 21 years of age and have a high school diploma or GEDMust be a mature, motivated person who is a team playerMust have proven verbal and written communication skills, the ability to work well under pressure, and a willingness to be flexible and adaptable as neededMust enjoy working in a fast-paced environment, talking on the phone with employees and clients daily, be gifted at working with various personality types and be able prioritize and able to produce positive resultsMust be able to follow verbal and written instructionsMust have excellent listening skills and the ability to ask probing questions politelyMust have good phone skills and excel at Customer ServiceMust be organizedMust be self-directed and be able to work with minimal supervisionMust be courteous and able to maintain confidentialityMust be competent using Microsoft Office and receptive to the use of technology to improve operations and overcome problemsMust have a positive attitude and exhibit patience and good judgmentMust have a valid driver’s license and the ability to maintain proof of current automobile insuranceMust have access to the internet outside of the officeMust be able to bend, stoop, and lift up to 25 lbs.Working Environment: This position is a 40 hour/week, non-exempt position, primarily working in the officeThis position requires regular evening and weekend commitments to be on-call, and for occasional office eventsThis position requires occasional holiday on-call assignmentsThis position will involve regular driving to clients’ homes and occasionally to meetingsThis is an administrative position but will require direct interaction with clients, families, employees and business associatesJob Type: Full-timePay: $20.00 – $23.00 per hourBenefits:401(k)401(k) matchingPaid time offRetirement planSchedule:10 hour shift8 hour shiftDay shiftMonday to FridayOn callWork Location: One location
