Job Description : Job descriptionFull Job DescriptionPosition SummaryAs a Care Manager, it will be your responsibility to build and maintain patient relationships throughout their care at Downtown’s Healthcare. You will be responsible for acquiring long term, quality patients for the clinic through the implementation of lead conversion systems. You will participate in educating the patients with the findings of an orthopedic exam, muscle palpation exam, diagnostic imaging and delivering a financial breakdown of the overall Care Plan. As a Care Manager, you will focus on understanding patient needs and wants, helping them overcome obstacles and empathizing with their struggles and challenges while influencing them to do what is in their best interest to improve their quality of life. To be a remarkable Care Manager, you must be able to develop trust and the confidence necessary to help patients navigate their Care Plan.
