Pay Range: $23.00 – $25.00 per hour
What You Will be Doing
We want people to join our team who share our values of love, excellence, respect, empowerment, and well-being. The ideal candidate will be involved in receiving and coordinating all referrals coming into the CME, managing reporting, and recording transitions of levels of care within the CME and exits from the CME. The Program Coordinators will conduct initial outreach to explain the service, obtain consent for participation, and link the individual or family/ caregiver to the appropriate care coordinator. Additionally, the Program Coordinators will serve as a liaison and support for individuals and families/ caregivers requesting enrollment in the 1915(c) to access available services, including submission of prior authorizations and assisting with the processes related to accessing flexible funds.
