Job Description : We are seeking a Remote Behavioral Health Case Manager with their CCM certification to join a well-known managed care organization as a Remote Behavioral Health Case Manager– CCM.
Must have active RN / LCSW / LPN / or LMFT in the state of HI to apply

Daily Responsibilities:
The RN / LCSW / LPN or LMFT will provide telephonic case management services to a specific clientele (retired veterans and active military members)
Contacts members of the medical team to discuss the patient’s course of progress and needs
Requirements for the Remote Behavioral Health Case Manager – CCM :
Must have active RN / LCSW / LPN / or LMFT in the state of HI to apply
CCM required
2+ years proven case management experience required in an MCO/Health Plan
2 years clinical psych experience in a health care environment
Hours for this Position:
M-F, 9:30am-6pm HST
Remote / work from home
