Job Description : Is responsible for the coordination of patient care including scheduling visits, maintenance, and upkeep of scheduling records and EMR systems. Timely communication of scheduling changes between office and field staff.*Must have Healthcare experiencePOSITION RESPONSIBILITIESMaintains a current client roster with necessary informationUpdates EMR system and schedules for clinical staffPrepares weekly schedule for field staff and provides notification to employeesAssists in coordinating services provided to patientsControls and monitors schedule changesChecks compliance of visits done (on master schedule) after all notes are matched to charges, brings any bringing problems to the supervisor’s attention immediatelyAssists in taking referrals, patient intake processProviding support to Payroll/Billing ManagerCalls hospital Social Services department to notify them that an Agency patient has been admitted to their hospitalObtains weekly visit count and report results to the Director of OperationsAssists in relaying messages to field staff, office staff, and community liaisons,Support HR department with recruiting needs, interview, and process new hiresOn-Call coordination gives and takes reports from the on-call callsJob Type: Full-timePay: $15.00 – $25.00 per hourBenefits:Health insurancePaid time offSchedule:8 hour shiftSupplemental Pay:Bonus payEducation:High school or equivalent (Preferred)Experience:Home Health: 1 year (Preferred)Work Location: One location
