Position : Patient Care Coordinator

Job Description : Patient Care Coordinator
The Patient Care Coordinator collaborates care between the facility, family, and providers that serve the patient. The patient care coordinator provides planning, direction, and coordination of overall healthcare of residents in our partner communities that elect our wellness program.
Supervised by: Atlas Administrator
Location: Wasilla, AK
Pay: $18-$20 an hour
Duties and Responsibilities:
Welcoming and interacting with residents with a pleasant demeanor and customer service attitude.
Collecting personal health information and inputting it into the electronic medical record.
Coordinating schedules of residents and providers to maximize efficiency.
Communicates with the resident, provider, and facility keeping him/her informed of all occurring events.
Provides Administrator with statistical data involving patients and delivered services.
Track patients progress of the continuum of care for 90 days post-acute stay in hospital. Refer to the Bridge program after 90 days if applicable.
Assist the partner community in any coordination of care that the patients might need.
Ensure compliance with federal, state and local regulations and standards.
Assist provider or facility with other duties as assigned.
Address and support cultural practices, as long as such practices do not harm others.
Adhere to all payroll, billing and documentation policies and procedures.
Professional Requirements:
Adhere to dress code, appearance is neat and clean.
Complete annual education requirements.
Maintain regulatory requirements.
Maintain patient confidentiality at all times.
Report to work on time and as scheduled and complete work within designated time.
Wear identification while on duty.
Attend annual review and in-services.
Attend staff meetings as scheduled and complete related educational activities.
Comply with all policies regarding ethical business practices.
Represent Atlas in a positive and professional manner in the community.
Communicate the mission, ethics and goals of Atlas.
General Requirements:
GED
1-2 year medical experience preferred
CNA License or MA License preferred but not required
Valid state driver’s license and reliable vehicle
Proof of current automobile insurance
Ability to relate well to staff and other professionals
Language Skills:
Ability to effectively communicate in English, both verbally and in writing.
General Skills:
Attention to detail
Basic computer knowledge
Proficient in administrative tasks
Excellent interpersonal skills
Familiarity with digital record systems
