Job Description : GENERAL SUMMARYCoordinate access and entry to Sound services across a wide range of referents and platforms. Provides cross system coordination and triage of services with local hospitals, school systems, legal systems and the community by acting as liaison and clinical subject matter expert in order to negotiate care needs. Navigate multiple database systems to facilitate entry into systems and verify criteria for services. Manage multiple tasks in an organized manner in order to adhere to critical timelines.ESSENTIAL FUNCTIONS AND DUTIESClinically screen referrals for appropriateness for care, the needs of the individual and the availability of necessary resources.Coordinate and facilitate case reviews within the agency, especially clinical supervisors, and link back to referents.Act as liaison to referral sources.Ensure financial eligibility by processing insurance verification for clients through Provider One and various state/county agencies.Verify client chart information, including but not limited to, client demographics, insurance coverage, appointment times/dates and all required paperwork.Monitor agency email and voicemail boxes related to clinical access, with all replies returned in less than 24 hrs.Communicate with outside sources to obtain client information and/or paperwork.Prepare data summaries and reports as necessary.Coordinate with other screening staff to enroll referents and act as clinical expert as needed.Perform other duties and responsibilities as needed and/or assigned.Provide all required case coordination services.Facilitate and develop cross-system coordinated service entry plans.Identify gaps in systems and service system barriers.Attend regular team meetings, program meetings, trainings and supervision as appropriate.Prepare all necessary written reports, summaries and progress notes as dictated by agency policy to be a part of the clinical record.Re-configure or re-negotiate treatment and services as appropriate.Comply with the agency’s clinical accountability policies and procedures and participate in quality improvement reviews, when assigned.Understand recovery concepts in behavioral health.Apply recovery concepts and synthesize and demonstrate recovery concepts.KEY WORK RELATIONSHIPS Establishes and maintains positive, ethical, and professional working relationships with the Sound management and team members, clients, tenants, government officials, inspectors, vendors, and other professionals. Relates to a wide variety of people including members of ethnic and sexual minority populations, and is respectful of individual and cultural differences.EDUCATION AND EXPERIENCEA master’s degree in behavioral health field, nursing or social work is preferred. BA is required.A minimum of two years clinical experience in a behavioral health field is preferred.Working knowledge and understanding of community resources and access to care standards preferred.Direct clinical experience with minority and low income populations preferred.Knowledge of community mental health systems, the public schools, State and legal systems and resources preferred.KNOWLEDGE, SKILLS, AND ABILITIESKnowledge of system resources and how to access them.Ability to communicate effectively and collaborate with community agencies/partners.Skills and experience working with diverse ethnic and cultural groups.Excellent communication skills, both written and oral.Strong attention to detail.Ability to establish and maintain effective working relationships within a team.Ability to work independently with minimal supervision, and the ability to multi-task while dealing with unexpected deadlines.Maintenance of high standards for accuracy, productivity, and confidentiality.CERTIFICATES, LICENSES & REGISTRATIONSMental Health Professional (MHP) designation preferred.Washington State driver’s license and insurable driving record required.Must be able to obtain a Washington State Agency Affiliated Counselor credential within the first 60 days of hire.Must pass a criminal background check.PHYSICAL DEMANDS While performing the duties of this job, an individual must be able to sit at a desk for three or more hours a day and use office equipment, including phones and computer keyboards. Individuals must be able to engage people over the phone and in-person by voice. Regular attendance is an essential function of the job.
