Job Description : The Case Manager works with emergency shelter program participants fleeing domestic violence to connect them to community resources and assist with ongoing safety planning and goal planning. Responsibilities include: conducting needs assessments; developing program participant-led goal plans; assisting program participants with housing and other important applications and documentation; transporting program participants to important appointments; delivering housing counseling and safety planning services for adults, children and families; representing the agency by participating in local, regional and state housing coalitions and work groups; and, maintaining client files and data entry systems including, but not limited to, case notes, service plans, and other important program participant documentation as required by funders.
