Job Description : The primary role of the Family Support Partner is to provide emotional support to youth and caregivers, instill hope, and help families build natural supports. The Family Support Partner ensures the youth and family’s voice is heard, and they help youth and families develop their ability to advocate for themselves throughout the high-fidelity wraparound (HFW) process. HFW is an evidenced-based care coordination process that has been demonstrated as an effective way to support children and youth with serious behavioral health challenges to live successfully in the community with their families. It is a process that identifies and builds on the strengths and culture of the youth, family, and support system to create integrated and individualized plans to address the needs of the youth and family that put the youth at risk of long-term residential placement. The family support partner (FSP) position is designed to provide intensive direct support for families.This position is a distinctly different job than the Family Support Coordinator but they work closely with the Family Support Coordinator to support positive outcomes for the family. Often, but not always, the family support partner is a graduate of wraparound and should always be a family member of a person with complex emotional or mental health needs and system experience. This includes helping the family understand how the HFW approach is likely different from other system involvement the family may have had previously.Support the Family Support Coordinator to do wraparound with the family, ensuring that wraparound is working for the family by individualizing the process to fit the family’s strengths and culture.Ensure the family understands the consent and release forms, and they gather information for the family that is utilized in the Strengths, Needs, and Culture Discovery.Support the family to identify their natural supports and prepare for the first team meeting.Help the team to understand the importance of, and feel comfortable with, family voice and choice and the family culture and strengths.Encourage creative thinking and advocates for options that are aligned with the family culture and strengthsEnsure the family understands the functional assessment and crisis prevention planning process.Check-in the with family regularly to ensure their voice is being heard, their plans are realistic, and, if necessary, helps them complete their action stepsExplore with the family the possible reasons that the implementation plan is not working and communicates with the facilitator about next stepsSupport the family to identify needs that will continue to require intervention after formal wraparound endsTake required training and work with a coach to demonstrate competency and fidelity to the wraparound principles and action steps (staff have 12 months after hire to complete their initial credentialing, and must complete re-credentialing every two years thereafter)RequirementsHigh school diploma or GEDA broad base of experience with life and possess a diverse view of what families need to have better livesHas lived experience caring for a child or youth with a serious mental health challengeAbility to be listen to and be an advocate for the families during this challenging timeHighly communicative and willing to engage people from different cultures, ages, and backgrounds, while establishing and maintaining healthy boundariesKnowledge of and experience with different systems, including schools, mental health, child welfare, juvenile justice, health, and othersKnowledge of local/regional community resources and an ability to engage theseHigh emotional intelligence and a growth mindset are essential characteristicsAbility to interact productively with individuals with multidisciplinary teamsProficient computer skills, including Microsoft Word, Excel, and OutlookExcellent organizational and prioritization skillsRequires travel throughout Routt, Moffat and Rio Blanco countiesAll Health Partnership employees must be fully vaccinated against COVID-19Preferred QualificationsBilingual (English/Spanish) language proficiencyA representative of the service population in demographic characteristics such as race, ethnicity, and languages spokenGood listening acumen, especially as it relates to youth voice and family systemsStrong ability to work effectively with across systems, both written and verballySelf-motivated with strong organizational skillsAbility to look at challenges as opportunitiesFlexible and adaptable of schedule to attend various meetingsAbility to read quickly with excellent comprehension and retentionHighly receptive to hearing feedback and striving to improve their skills as a family support partnerTeam PlayerEngage in cross-organization efforts, connecting project work to the broader PartnershipShare unique skills and expertise with NCCHP teamCompensation1.0FTE plus benefits;PTO, health & dental insurance, 401(k) and disability insurance benefits$45,000 per yearJob Type: Full-time
