Job Description : WASATCH HOMELESS HEALTH CARE, INC.Job DescriptionPosition Title: Quality Improvement and Risk Management AnalystDepartment: Clinical OperationsSupervisor: Director of Clinical OperationsStatus: Full-Time (40 hours per week)Amount of Travel: Limited local travelPosition Overview: The Quality Improvement and Risk Management Analyst serves as a member of the clinical operations team that is responsible for collecting, organizing, monitoring, and distributing information related to quality and risk management functions, including but not limited to compliance and industry standards. The Quality Improvement and Risk management Analyst oversees the tracking of patient complaints/feedback and incident reports, attending various monthly committee meetings to assist in presenting the data. Additionally, the Quality Improvement and Risk Management Analyst is responsible for generating reports, reviewing and analyzing data, as well as reporting on data outcomes to various departments.Core Responsibilities: Quality ImprovementProvide support to key staff in areas related to Quality Assurance and Quality Improvement (QA), which may include identifying, implementing, and monitoring QA/QI initiatives across the organization. Work in collaboration with the Director of Clinical Operations on these QA projects.Share and promote quality improvement initiative with all clinic staff.Assist with creating data dashboards that are reviewed at various Quality Assurance workgroups and Board meetings.Ensure compliance with quality data relating to UDS (Uniform Data System) collection and reporting requirements related to clinical data.Assist with grant-funded clinical quality improvement efforts with key staff; collaborating, supporting and assessing data needs for such grants.Ensure proper compliance with regulatory agencies, accrediting bodies, and policies and procedures. Comprehend and report any changes to the Quality requirements/regulations and assist in reviewing policies to reflect such changes.Collaborate with key staff in conducting patient satisfaction surveys and report summary of findings. Evaluate areas of improvement for patient experiences.Risk ManagementProvide support by responding to incident reports, researching possible adverse events, and assist in creating action plans.Conduct regular risk hazard assessments.Identify and address potential issues that may arise from incidents.Support organizational risk management goals and become an expert in policies and procedures to negate further risk.Assess, investigate and research patient complaints and incidents and address questions or concerns.Ensure accurate and timely entry of patient complaints and incidents into the Risk Management log for review by the Risk Management Committee.Under the direction of the Risk Management Committee, conduct any necessary follow up investigations to gather information for evaluation of incidents and patient complaints. Serve as primary person for investigating or following up on incident reports and/or patient complaints.Assist in creating risk management incident dashboards that are shared at various internal and Board meetings.Participate in Risk Management trainings and certifications, as needed.QualificationsUndergraduate degree from an accredited college in data analytics, information management, risk management or other closely related field, plus 2 years of work experience in related field, or a combination of related education and experience.Strong oral and written communication skills. Ability to present in various group settingsAbility to manage excel sheets and data.Ability to track and collect date to measure progress towards program goals and outcomesUnderstanding of risk stratification and analysis.
