This individual supports their patients in accessing the resources or tools needed to reach specified goals, relating to behavior modification, self-management, and/or resource acquisition. The Support Coach plays a hands-on role, serving as an advocate, a role model, and a guide in a patient’s journey for more stability and wellness. This individual is, or can be, Peer/Family Certified, meaning they can identify personally to accessing supportive behavioral health services for themselves or for personal contacts, and understand the value and challenges in navigating the systems of care available to our community. The Support Coach can also read and understand clinical documentation to conceptualize how to address the specific goals they are assigned to work with the patient on. The Support Coach will utilize evidence-based intervention models and community resources to ensure that the services provided are best practice for the patient. Services may be provided in office, in the home, in the school or in the community, as well as virtually. The Support Coach may have up to 45 active patients on their caseloads.** Covid-19 Vaccination Required Within First 30 days of Employment***Reasonable accommodations for employees who are exempt based on ADA or Title VII.Locations: East Valley–334 W 10th Pl, Mesa AZ 8501MINIMUM QUALIFICATIONS18 Years of age or older; ANDBachelor’s or Master’s degree in behavioral health (B.H.) related field; orClinical Intern in Bachelor’s or Master’s degree behavioral health program in good standing; orBachelor’s or Master’s degree in non-BH related field AND 1 year of BH work experience; orAssociate’s degree AND 2 years of BH work experience; orHigh School diploma and 4 years of experience in a BH setting (1 year must be direct patient contact).
