Job Description : THE SALVATION ARMYINTERMOUNTAIN DIVISIONCase ManagerName: N/A Unit/Dept.: Lambuth Family Center Job Title: Case Manager FLSA Status: Full Time, Non-ExemptReports to: Program Supervisor Schedule: Varies Supervises: N/A Start Date: TBDAll Salvation Army employees working at a facility covered by this order (Denver City Covid – 19 High-Risk Setting Requirement per Mayor Hancock) must be fully vaccinated. Function:This is a full time, non-exempt, hourly position, 40 hours a week with vacation and sick time as well as medical, dental, and optional insurance coverages.The program seeks a candidate who is flexible and available to work 8-hour shifts. This position provides intensive case management Schedule to be determined based on coverage needs. The assistant works with guests to ensure that all are being safe, responsible, and respectful while at the facility. We are seeking a candidate who can work independently with limited supervision. This position requires the ability to use Trauma Informed Care and Motivational Interviewing skills. This position requires the ability to use discretion when addressing or resolving resident matters while providing a positive customer experience for program guests. The assistant requires basic knowledge of record keeping practices and procedures. This position also has some cleaning responsibilities in addition to light food preparation/serving.Duties and Responsibilities: Ensure all Salvation Army policies are observed while directly assessing the needs and goals of the families.Manage intake and case plansProvide case management services with the goal of moving toward self-sufficiency and stabilityDetermine the appropriate agency or referral site for individual needs and act as an advocateConduct one time and ongoing assessments including Herth Hope Index, Self Sufficiency Matrix, and Vi-SPDATManage case files: organization and maintenanceAssist regular programs established by scheduleAttend scheduled staff meetingsOther duties as assignedQualification and Education Requirements: Education and Work Experience:· Bachelor’s Degree in Human Services field required, Master’s Preferred· Previous volunteer or work experience with homeless populations, non-profits, ect.Knowledge, skills, and abilities required· Knowledge of Trauma Informe Care and Motivational Interviewing· Ability to work with minimal supervision· Ability to use discretion and independent judgement when addressing or resolving client matters· Ability to adhere to HIPPA standards of confidentiality· Knowledge of community resources and agencies· Ability to communicate effectively with staff and clients· Knowledge of the professional code of ethicsSoftware Skills:· Microsoft Outlook, Word, and Excel required· Ability to use new software programs with basic training· Be supportive of The Salvation Army’s mission.
