Job Description : Infant & Maternal Resource Coordinator

The Infant & Maternal Resource Coordinator is primarily responsible for helping target populations navigate & access YMCA and maternal health programs & resources, in order to work toward adopting healthy behaviors. Please note: this 10-month position is grant-funded, and continued employment is contingent upon ongoing grant funding.

Starting salary for this position is $24.00/hour. Benefits for FT employees include free family membership, health & dental insurance, time-off benefits, program discounts, and optional retirement savings plan.

Responsibilities:
Assess eligibility and enroll participants in assigned programs
Provide basic family and individual facilitation; work with individuals to develop prioritized pathways.
Coordinate and follow-up on referrals to other community organizations that specialize in areas prioritized by individuals.
Coordinate services to improve access to services for participants by directly assisting clients in identifying barriers to improving their health and wellness.
Enter participant data in the assigned program database per protocol, including demographics, eligibility/enrollment status; update as needed.
Identify and document system/service barriers related to connecting participants with health and program services.
Perform program coordination work with minimal supervision. May be required to occasionally work irregular hours.
Qualifications:
Bachelor’s degree in wellness, social work, or related field preferred.
Good organization/planning, team building, verbal and written communication skills necessary.
Requires excellent networking, training, communication and interpersonal skills.
Successful experiences working with people from diverse backgrounds.
Ability to establish & maintain harmonious relationships with staff, volunteers, members & the general public.
