The Young Adult Oncology Patient Navigator is a team member of Supportive Oncology Services (SOS), providing supportive care at the UC Davis Comprehensive Cancer Center. This mission of SOS is to serve people whose lives are affected by cancer, through comprehensive assessment and expert management of the myriad of challenges they face.

Responsibilities
The role of the Young Adult Oncology Patient Navigator is to provide support, education, referrals, and resources to help navigate young adult cancer patients (18-39 year-old) throughout their care within inpatient and outpatient adult oncology care settings. Under general direction and team collaboration, the Young Adult Oncology Patient Navigator conducts needs assessments, identifies barriers to care, coordinates efforts to reduce barriers by connecting patients to UCDH and community services, facilitates logistical needs to improve treatment adherence, and provides education, guidance and support to facilitate fertility preservation when indicated
Required Qualifications
Experience working with the general public and community agencies in collaborative and professional settings
Experience with multi-cultural, multi-generational families, including non-traditional family structures
High customer service and cultural humility orientation
Skilled in related to multi-disciplinary team, able to promote and engage in relationship building with healthcare teams, patients and caregivers
Strong professional boundaries, able to work collaboratively utilizing a team approach
Excellent communication ability, including positive interpersonal skills working with diverse people
Proficient computer skills, including Microsoft Word, Excel, Power point.
Ability to curate computer searches and distinguish safe valid and vetted information from reliable sources
Preferred Qualifications
Bachelor’s degree, preferably in the fields of social work (BSW), psychology, health care or human services
Bilingual fluency – Spanish, Russian, Tagalog or Vietnamese
Medical/health care experience
Familiarity with oncology knowledge, cancer-related psychosocial impact
Experience teaching reproductive and sexual health, fertility and relationship health
Special Requirements
This position may be subject to a criminal background investigation, drug screen, Live Scan fingerprinting, medical evaluation clearance, and functional capacity assessment.
