Job Description : All applicants must be fully vaccinated

Will complete home visits in the community in all boroughs to complete assessments face to face; document the outcome of the attempts at completing the visits, by writing notes which will follow a specific format. Carry out phone outreach to clients, collaterals and third party providers in the supervision of case management services as needed.

Requires a degree in Social Services (social work, psychology, sociology, etc.).

All work is done remotely at this time. Hours hopefully can be a minimum of 15-20 hour per week.
