Job Description : Please include cover letter.Summary:This full time position is responsible for providing direct support services to and counseling to the most chronically homeless, unsheltered individuals in our community. The priority for this position is to identify, engage, assess and intervene with the above named population, with a goal of assisting these individuals into permanent housing. This position is supervised by the Outreach Manager and Deputy Director.Responsibilities:Provide direct outreach activities to identify homeless, unsheltered individuals in Leon and Gadsden county. Tasks will include visits to encampments, local parks and other areas to identify and engage participants.Distribute supplies and food to unsheltered individuals.Utilize in house rapid rehousing funds and other resources to assist eligible consumers into permanent housing.Assess consumer needs, develops independent living plans, and arranges for related services as needed.Participate in ongoing case staffing with Coordinated Entry Team staff to prioritize those identified for CoC funded services, such as Permanent Supportive Housing and Rapid Rehousing/Intervention resources.Provide crisis counseling, advocacy, independent living skills training and other supportive services to persons with disabilities.Maintains all appropriate program and consumer documentation, including COMS/HMIS data entry/documentation.Provide general information and referral to those experiencing homelessness and persons with disabilities.Required BA/BS in social work or human services field, with minimum of two years of related work experience in providing direct services to persons experiencing homelessness. Candidate should possess strong verbal and written communication and organizational skills. Qualified persons with a disability are encouraged to apply.
