Position : Program Coordinator

Job Description : Rehabilitation Support Services, Inc. is seeking a full time Program Coordinator to run the daily operations of our Supportive Housing apartment program in Liberty, NY.Job responsibilities include, but are not limited to the following:ESSENTIAL FUNCTIONS:1. Supervision of the operation of the Chestnut St Apartment Program.2. Ensure the Protection of all Client Civil Liberties, Rights and Property3. Ensure all Policies and Procedures are carried out.4. Ensure leases are up to date and renewed when due. Maintain a relationship with landlords.5. Direct Supervision/ Monthly Supervisions of Supportive Apartment Staff6. Monitor Staff trainings and ensure all certifications are up to date.7. Staff interviews and hiring8. Admission screenings9. Complete Discharge Summaries and follow up notes (30,60,90 day)10. Review Service Plans for the program and work closely with staff creating the plans for their caseload.11. Review all notes/documentation upon the completion of the Manager’s review.12. Lead Staff Meetings and maintain minutes.13. Performance Evaluations (6 months, yearly) for Supportive Apartment Staff14. Incident Reports for Supportive Apartments15. Annual Reports for Supportive Apartments16. Responsible for the Petty Cash17. Participate in On Call Responsibilities18. Represent the Agency at SPOA Meetings19. Assure all benefits are in place and manage receivables and PNA check requests.20. Complete Representative Payee reports when due and address social security issues as they arise.21. Ensure medication rotation is completed daily22. Meet with QI staff on a monthly basis to address any issues.23. Ensure that there is regular contact with Service Providers and recommendations are carried out. Attend team meetings when necessary.24. Perform other duties as assigned by the immediate Supervisor/Director.Requirements:High School Diploma and 5 years of experience in human services, or Associate’s Degree and 3 years of experience in human services or Bachelor’s Degree and 1 year of experience in human services.Must be computer literateActive/Valid driver’s licenseSuccessful completion of a criminal background check including Justice Center and New York State Office of Children and Families Statewide Central Register Database clearanceBenefits:The agency offers excellent salary and benefits package including medical, dental, vision, flexible spending account and 401K as well generous accruals including paid sick, vacation, personal and holiday time.
