Job Description : About this position:The Medical Records Manager works under the direction of the Executive Director to ensure all medical records are managed in accordance with Company policy and state and federal regulations. The Medical Records Manager is responsible for the management, completion and accuracy of medical record information kept in both written and electronic forms and is responsible for ensuring that all protected health information is safeguarded as required by Company policy and HIPAA. The Medical Records Manager works closely with the nursing department to ensure accurate and complete medical records are kept for all residents.Minimum Requirements:Must pass a Background Check (Reference Check, Drug Testing)TB ClearanceEmployee Health Screening Post HireMust be able to speak and understand EnglishJob Specific Duties:1 Know, understand and implement the mission of Haven Health2 Follow all company policies as contained in the Employee Handbook, Code of Conduct and all other active policies including but not limited to attendance, dress code, workplace safety, zero-tolerance policies and professional conduct.3 Participate in all required meetings, trainings and events.4 Maintain an accurate resident census by resident name and room number.5 Maintain accurate list of emergency telephone numbers.6 Maintain filing system for health records that meets the needs of the facility, including overflow files for current residents and health records of discharged anddeceased residents.7 Confirms that all appropriate transfer and admission paperwork has been completed; contacts transferring facility or entity for additional information as needed.8 Requests the history and physical exams for residents and obtains an updated and original signature.9 Properly labels all physical charts in use and ensures that all forms are completed and ready for use.10 Collects and processes all telephone orders for mailing; ensures that all orders are signed by physicians and dated before updating them in the medical record; dailyreviews the telephone orders and admit orders for accuracy.11 Ensures that nursing stations have all needed forms and information in order to properly update the medical records.12 Assembles discharged residents’ medical records for storage / archiving and ensures all records are complete; ensures that the record is completed and ready forstorage within 30 days of discharge.13 Reviews physician visit log and or records of attending physicians and progress notes and prepares a list of resident are due to be visited by their physicians;distributes these lists to nurses stations and physicians; enters visits in the clinical software.14 Reviews clinical software dashboard for due MDS assessments and care plan reviews; monitors that MDS and assessments (UDA) are completely timely.15 In conjunction with the Nursing Department and the Business Office Manager, code all diagnoses on admission, throughout the resident stay and upon discharge foreach resident using ICD-9 coding system; distributes proper diagnoses and code numbers to appropriate departments.16 Controls the release of medical information to authorized persons when appropriate requests are received and upon approval of the Executive Director. All legal,CMS, Humana and other Third-Party audit requests are to be reported to the Health Information Director.17 Ensure the electronic medical record is maintained by: monitoring dashboard information and informing the appropriate staff members of missing information andthen enters and updates the correct information; enters all physician orders upon admission if not completed by nursing; review draft order and schedule; enter allphone orders and other updated information daily; print monthly physician order recaps the first day of each month; ensure that all monthly physician orders aresigned by the physician in a timely manner; audit the EHR per the schedule and report findings to the Quality Assurance Performance Improvement committee.18 Communicate needed information to lab and pharmacy services as needed.19 Track and obtain certification and recertification physician signatures as applicable in a timely manner.20 Complete all other assigned tasks and responsibilities as they pertain to this position as well as any other responsibilities you have been delegated and cross-trainedto perform.Job Type: Full-timePay: $16.00 – $20.00 per hourSchedule:Monday to FridayExperience:Medical records: 1 year (Preferred)Work Location: One location
