Job Description : Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation’s largest dental residency program, and New York State’s largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs. Position Summary:
We have an exciting opportunity to join our team as a Program Counselor, Required Spanish – Brooklyn.

In this role, the successful candidate The counselor is responsible for providing support for patients and community members.. This includes outreach, workshops, intake, screening, assessment, advocacy, assistance in accessing FHC health services such as vaccines, and case management services.
Job Responsibilities:
Provides outreach and conducts workshops
Responsible for screening of community members for needs and for public benefits, such as SNAP, unemployment, cash assistance, etc
Assists clients with benefit applications
Makes referrals to a variety of public and community agencies providing food, shelter, clothing, medical, educational, and other services.
Assist community members in accessing health and behavioral health services within the FHC such as vaccines.
Complete documentation in EPIC and ETO as required.
Attend staff meetings and other required meetings.
Counsels clients on available resources and provides advocacy.
Strengthens relationships with community partners to identify resources for clients
Minimum Qualifications:
To qualify you must have a High school diploma required.
At least two years experience working in health care or community based organization setting if only has a high school degree and one year experience if has a BA
Preferred Qualifications:
Excellent communication skills, ability to multi-task, experience in providing services to diverse community, ability to be a team player.
Qualified candidates must be able to effectively communicate with all levels of the organization.
