Job Description : Masters-Level Behavioral Health Case ManagerThis is an opportunity for an experienced, Masters-level Case Manager to join a small but growing substance abuse recovery outpatient facility in Gilbert (moving to Mesa) that works exclusively with Native Americans struggling with addiction and trauma.This highly experienced Case Manager will collaborate with a team of Clinicians and Peer Support Specialists to assist Native American clients in their recovery goals and routine life needs. Desert Sage Healing clients are both outpatient and residents of behavioral group homes.As Case Manager you will also help ensure that our practice strictly complies with both the letter and the spirit of State of Arizona and AHCCCS requirements, guidelines, and regulations.As Case Manager you will be responsible for designing and managing the portfolio of client requests related to routine recovery and life management goals. For example, you will help execute in a predictable and efficient fashion all requests for government identification, medical appointments, job-related appointments, education and life-skills development related appointments, etc. You will also be able to recognize when client requests are unreasonable or should be fulfilled by the client themselves or by their group homes when relevant.You will be dependable, caring, compassionate and punctual. You will have excellent written, verbal interpersonal and problem-solving skills. You will be versatile, flexible, and willing to work within constantly changing priorities with enthusiasm. This is a challenging role but with long-term upside. The ideal candidate will understand that they are a part of a team that is still forging culture and is comfortable with change.Desert Sage Healing can provide supervised hours toward licensing for the relevant candidate.Routine responsibilities include: Coordinating care with residential behavioral group homes to ensure clients are having adequate and reasonable assistance in meeting their recovery and life goalsEmpowering clients to advocate for themselves and gain experience in owning success in their goalsRegular, scheduled (2x week) trips to outside services centers for all services that cannot be obtained online or by phoneMust-Have RequirementMaster’s degree by accredited school in relevant fieldMinimum of 2 years direct and recent experience, preferably in the outpatient behavioral health fieldAbility to self-manage as a book of business all case management activities with limited oversight by managementA schedule that allows you to in our office Monday through Friday from 9am to 3:30pm with NO routine interruptions or outside distractionsPreferred RequirementsMinimum of 3+ years’ experience in relevant Behavioral Health disciplinePractical, working experience with AHCCCS behavioral health, resources, policies, requirements, and guidelines; background check; minimum three year’s recovery from drugs and/or alcohol if relevantExperience with Native American clients a big plusJob Types: Full-time, Part-time, ContractPay: $28.00 – $32.00 per hourBenefits:Flexible schedulePaid time offMedical Specialty:Addiction MedicineSchedule:8 hour shiftMonday to FridaySupplemental Pay:Bonus payEducation:Master’s (Required)Experience:case management: 2 years (Required)Work Location: One location
