Job Description : Position Description: Hospice Maui, a local, non-profit, legacy hospice organization, was founded on Maui over 40 years ago to offer high-quality, end-of-life care to patients and their families. On February 1, 2022, services were also expanded to the islands of Moloka’i and Lana’i..Position Summary: Hospice Maui is seeking an Outreach Liaison to join our Advancement team to educate healthcare providers and the community about Hospice Maui’s programs and services. Liaisons will also participate in special events, agency-wide strategic planning, and long- and short-term departmental planning.in furthering the mission of Hospice Maui.Essential Job Functions/ResponsibilitiesActively manages physician referral base, making regular visits with medical practices and post-acute community agencies. Creates awareness of Hospice Maui and our services through one-on-one contact.Maintains an up to date record of activities on tracking tools including CRM, Excel, Google Sheets.Researches, identifies and develops connections to new physician and medical provider resources in our service area.Provides updated collateral material to potential and current referral sources.Plans, organizes, and participates in community events & recognition activitiesPeriodically visits community partners including business, organizations, educational and religious institutions to provide Hospice InformationAssists with development of material and presentations to be used for community education, events and programs.Participates in conferences, health fairs and health care community events to increase Hospice Maui’s presence in the community.Communicates all pertinent issues with Outreach Team Lead.Organizational Responsibilities include:* Promotes positive co-worker and teamwork relationships* Supports the goals and mission of our organization* Maintains professional appearance, behavior and attitude* Maintains confidentiality and ethical behavior* Maintains a team approach through cooperation, mutual respect, flexibility and accountabilityNote: The above information is intended to describe the general nature and level of working being performed by the person in the job. Management has the right to add to, revise, or delete information in this description.Position Qualifications (Knowledge, Skills and Abilities)EducationAssociates degree (required); Bachelor’s degreeExperience1 year prior hospice and liaison experience (required), 3+ years prior hospice and liaison experience (preferred)Familiarity with the medical landscape in our service areaStrong communication, presentation, and interpersonal skillsDemonstrated networking abilitiesProficient in Word, Excel, and PowerPoint (required)An equivalent combination of education and experience, which provides proficiency in the areas of responsibility listed above, may be substituted for the above education and experience requirements.License/CertificationRN, LPN or Social Worker licensure (preferred but not required)Valid driver’s licenseOther QualificationsAbility to successfully relate to a diverse population and workforceAble to professionally and effectively interact with internal and external contactsAble to work a flexible schedule to enable community engagement on weekends and after hoursPhysical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
