School based experience preferred.Knowledge of how to access community resources.What You’ll Do:Student and Family Services: Provide case management services to youth and families including: comprehensive assessment, service plans, referrals and follow-up. Link families with needed health, mental health and other resources in the community.School & Community Services: Work collaboratively with the Beacon/Community School Manager and site administrator, participate in student assessments.Administrative Duties: Develop and implement processes for referrals and provision of case management services, maintain accurate and up-to-date clinical records and other documentation, Participate in meetings as required.Job Purpose:To promote healthy development and positive outcomes of youth and their families through case management, resources linking and advocacy strategies in a San Pablo elementary school.
