Position : Admissions Coordinator
Job Description : Primary Purpose:Coordination of admissions to the Health Center at Skyline. Responsible for gathering all information necessary to evaluate potential admission of referrals from hospitals and other provider organizations to ensure the referral meets community clinical admission criteria and facilitate optimal transition of care. Serves as primary point of contact for internal transfers from Skyline independent living, assisted living and memory care into Health Center. Gathers history and physical and other clinical information as well as payer and insurance information, and obtains and documents necessary authorizations to ensure a smooth and successful admission process. Develops and maintains referral source relationships to achieve/sustain a quality and quantity of referrals to achieve established occupancy goals.Principle Responsibilities:ADMISSION PROCESS· Maintain familiarity with current guidelines for Medicare, Med Advantage plans and long term care insurance.· Coordinate with Director of Nursing, administration and billing departments to maintain written admission guidelines with clear and consistent admission criteria, policies and procedures.· Manage admission inquiries including phone inquiries, general emails, incoming faxes to ensure prompt follow-up and avoid losing potential admissions· Obtain information regarding prospective admissions by requesting completed applications and medical information; verifying and clarifying information; interviewing patients and family members; explaining admission criteria.· Compare patient’s documented condition to admission criteria and provide recommendation to Director of Nursing for accepting or rejecting patients; refer patients and family to other programs and institutions when Skyline admission is not available.· Communicate with other departments as necessary (e.g. therapy, social services) in relation to the assessment process.· Collect and compile all information necessary to establish the prospects admission record in the Electronic Health Record, including the Common Working File and Verification of Funds.· Enter information into Electronic Health Record and upload necessary admissions documents in an accurate and timely manner· Counsel residents and family members with Medicare and other insurance questions when needed.· Communicate closely with Transforming Age billing office to ensure necessary information is provided.· Admit patients in accordance with state and federal guidelines by completing admission packet including financial responsibility forms.· Provide optimal response time to referral sources to achieve customer satisfaction and maximize successful admissions.· Arrange and schedule medical transportation services for new admissions when needed through the established request system.· Establish contact with new resident upon arrival, introduce to room and call system, introduce key staff.· Create a positive resident experience that is resident centered, from the referral through the discharge.· Resolve patient/family concerns regarding admission process and recommend changes in service policies and procedures.What we’re looking for from you: Education: Bachelor’s Degree in Social Work or in a related Human Services field from an accredited college or university; or combination of education and work experience.Experience: 1 plus year of experience in community based programs for senior population.Knowledge: Must be knowledgeable in Medicare, Med Advantage plans and LTC Insurances.
