Position : Evening Case Manager

Job Description : NJRC is seeking out a Case Manager to enhance the quality of patient management. The successful candidate will be accountable for the full patient care cycle. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed. A certified case manager is a plus.CADC or CPRS required.ResponsibilitiesMaintaining the confidentiality of all patient and facility activities.Conducting Intakes for new patients.Completing D/C paperwork.Assisting clients in obtaining necessary resources to achieve employment goals.Coordinating care with any outside providers that the client has appointments with.Maintaining client related data systems, including case notes and complete KIPU entries.Preparing case management outcomes including but not limited to discharges and clients leaving against medical advice.Completing case management needs assessment on all clients assigned to Case Manager.Developing appropriate individual plans with clients as the means for implementing appropriate services.Providing on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the treatment plans and needs assessments to assist the clients in achieving the stated goals and objectives.Providing ongoing face to face reviews with the client on a regular basis to assess the progress made in reaching goals so that the treatment plans can be modified as necessary to ensure that the goals and objectives are being achieved.Documenting all contacts within 24 hours of contact including face-to-face, collateral and networking contacts.Demonstrating appropriate & complete documentation of treatment – related information including aftercare plans and reviews, correspondence, releases of information, etc.… in compliance with agency standards and regulations.Working with clients to develop individualized case management plans.Resolving conflicts through general and supportive advice.Preparing educational and motivational lectures.SkillsProven working experience in case management, including as a nurse, medical, mental health care manager or a related jobExcellent knowledge of case management principles, healthcare management and reimbursementPrevious experience with psychological aspects of careEffective communication skillsExcellent organizational and time management skillsFamiliarity with professional and technical emerging knowledgeProblem solving skills and ability to multi-taskCompassionate with teamwork skillsCurrent license, certification or registrationBS degree in related healthcare fieldScheduleMonday, Tuesday, Thursday evening (roughly between 5:00-9:00 p.m.)9-12 hours weeklyJob Types: Part-time, ContractPay: $25.00 – $35.00 per hourExperience:Case management: 1 year (Preferred)Work Location: One location
