The CDC Foundation seeks candidates for a Linkage to Care Coordinator. Under general supervision, the Linkage to Care Coordinator will provide education, outreach, and linkage to care for people with opioid use disorder, substance use disorder or co-occurring disorders. The Linkage to Care Technical Assistance Coordinator will identify opportunities and barriers to accessing care and work with existing linkage to care staff to provide guidance to overcome challenges, create effective processes and increase collaboration.This position is local, in person, and is in either Trenton, Newark or Ewing, New Jersey.POSITION TITLE: Linkage to Care CoordinatorLocation: This position is local, in person, and is in either Trenton, Newark or Ewing, New JerseyPosition End Date: 7/31/2022Salary: $65,000 – $75,000, Plus BenefitsQualifications:Minimum of five (5) years of full-time, professional work experience with linkage to care, care coordination, program and case management relating to substance use disorder.Bachelor’s degree in Public Health, Social Sciences, or related field. Applicants who do not possess the required education may substitute additional experience on a year-for-year basisPeer Recovery Specialist who has met all the requirements of the CPRS, RCP or NCPRSS certifications.Demonstrated knowledge in overdose prevention, Substance Use Disorder treatment, harm reduction, recovery, and/or drug policyMust have a minimum 5 years of recovery from lived experience in substance use and/or co-occurring mental health disorder(s) and demonstrates a passion for tackling the opioid epidemic.Demonstrates understanding of and competence in serving culturally diverse populations.Experience providing training relating to Substance Use.Experience utilizing trauma-informed and strengths-based approaches.Highly motivated and organized with the ability to work well in teams and independently.Strong written and oral communication skills, with ability to be efficient and consistently deliver high-quality work under time constraints.Ability to coordinate and collaborate with diverse stakeholders.Knowledge of community resources preferred.Demonstrated ability to work well independently and within teamsExperience working in a virtual environment with remote partners and teamsProficiency in Microsoft Excel, Word, PowerPoint, Teams and Zoom. Knowledge of Recovery Data Platform preferred.Preference will be given to applicants who currently are active in the field of Substance Use Disorder(s) and Recovery, and who are fluent in the continuity of recovery oriented systems of care, along with a history demonstrating commitment to addressing the opioid epidemic.Responsibilities:Provide assistance to first responders in all 21 counties in NJ with outreach, engagement, referrals, warm hand offs, and care coordination services for individuals with substance use disorder and co-occurring disorders.Advise on evidence-based and accurate substance use disorder education to individuals, groups, and organizations.Create a training curriculum and outreach materials for first responders and community stakeholders on best practices and resources for linkage to care.Training and education to first responders on best practices and resources for linking individuals they encounter to care.Develop networks with external stakeholders and service providers to increase access to community-based resources.Provide assistance with assessing and managing referral processes to address necessary challenges for patients and providers.Coordinate with community partners on access to evidence-based supports and programs, including additional non-medical needs of individuals (for example, housing, employment, insurance, food, internet, etc.).Support persons engaged in care.Assist with conducting outreach to engage or re-engage individuals in care.Participate in evaluation and quality assurance activities.Access to personal transportation preferred to assist with performing job duties.If position requires local travel to serve or transport participants, employee must pass pre hire and annual motor records check and carry minimum auto liability coverage.This role is involved in a dynamic public health program. As such, roles and responsibilities are subject to change as situations evolve. Roles and responsibilities listed above may be expanded upon or updated to match priorities and needs, once written approval is received by both the CDC Foundation and New Jersey Department of Health in order to best support the State of New Jersey in their public health programming.The CDC Foundation requires all employees to be fully vaccinated* for COVID-19 as a condition of employment effective November 22, 2021.
