Position : Program Specialist

Job Description : SPECIFIC DUTIES AND RESPONSIBILITIES:Administrative1) Coordinates and assists in the development and implementation of the Individual Support Plans (ISP) and to documentation of progress2) To support individuals in their growth towards greater independence in self-care, daily living skills, socialization and community living3) Responsible for ensuring all services are documented in accordance with regulations and agency policies and procedures4) Reviews daily notes to ensure staff compliance with outcomes5) Completes monthly and quarterly reports on each individual assigned to the caseload6) Attends professional development sessions monthly and maintains required certifications in compliance with the applicable State Regulations7) To support, supervise and provide assistance to programs in reference to goal and plan implementation and training in programmatic issues8) Ensures waiver renewal with the Medical Assistance Office9) Ensures AIS/SH/HCH is applied for as needed and that renewal applications are reviewed by the team and completed one month prior to the expiration of the service10) Performs work assignments with minimum need for supervision and guidance11) Originates actions and actively influences events. Offers suggestions without waiting to be asked12) Demonstrates ability to set goals, assign priorities, anticipate problems, establish limits, and achieve resultIndividual Care1) Participates in the development and implementation of Individual Service Plan (ISP)2) Completes all intakes of new individuals3) Prepares for, schedules, conducts, and attends all quarterly meetings4) Attends and participates in all annual ISP meetings5) Develops assessment and updates Lifetime Medical History and protocols6) Ensures all residents receive required and necessary medical/dental/psychiatric/all other care/appointments in a timely manner and ensure follow-up occurs on all recommendations by the individual’s health care provider7) Participates in Systems Intensity Scale (SIS) assessments when conducted and ensures informants are present who have knowledge of the individual’s skill levels8) Participate in Consumer Data Collection (CDC) when conducted and ensures all medical records are available for review, including archived records if necessary9) Provides overall case management and advocacy for all individuals10) Assists the individuals in the expression and fulfillment of their needs through the development of a community life plan, its schedules and updates for each individual11) Understands the purpose of adaptive equipment; makes recommendations as to adaptive equipment that would allow individual to be more independent and functional; insures adaptive equipment is available and used by the individual.12) Treats all people with dignity and respect13) Takes into consideration the needs and position of othersOperations1) Responds in accordance with agency policies and procedures for handling emergency situations2) Conducts and participates as a team member in supervisor, staff and team meetingsReporting Responsibilities1) Completes files and maintains all required documentation which includes monthly summaries, quarterly and annual ISP reports, incident reports, and other related forms as directed by supervisor2) Acts as HCSIS Point person and enters and investigates incident reports3) Updates all demographic and medical emergency information in individuals’ records4) Required to complete and pass the Certified Investigator’s training and complete and pass the renewal process every 3 years.5) Participate as an active member of the monthly Incident Management Review Team and ensure completion of incident documentation according to Incident Management guidelines.Corporate Compliance1) Reports to work, meetings and appointments in a punctual mannerprepared to work.2) Keeps supervisor well informed of status of assignments.3) Responds to requests and leadership of supervisor4) Helps supervisor get support for departmental goals and plans5) Supports and follows all agency policies and procedures as well as complying with all applicable State regulationsQuality Assurance1) Ensures that the programs, vehicles, and equipment are safe, functional, and secure for the individual’s physical well being2) Manages relationships with individuals, families, team members, staff, advocates and neighbors by responding to inquiries with tact and sensitivity.3) Demonstrates a courteous, cooperative attitude while maintaining strict standards of confidentiality and professional conduct4) Works effectively with others towards a common goal.5) Establishes and sustains a positive impact on people.6) Actively and positively contributes to morale and teamwork.7) Achieve quality resultsSupervisory ResponsibilitiesN/AOther Duties1) Participates in on-call responsibilities2) Performs other duties and responsibilities as assigned3) Works overtime as necessaryJob Type: Full-timePay: $50,000.00 per yearSpectrum Community Services Inc. is an Equal Opportunity Employer.
