Job Description : The Community Action Committee of Pike County has a position opening for a Patient Care Coordinator.Essential Functions…Functional Area: Care Management, Referrals, Follow UpEstimated Effort: 60%Responsibilities, Deliverables, Outcomes and ExpectationsComplete Pre-Visit Planning (including add flag in ECW, notes in Azara) for each client one week prior to each client’s appointment.Participate in Morning Huddles.Facilitate coordination between Medical, Dental, and Behavioral Health to promote integrated care.Provide education/support in self-management of health issues.Schedule and follow up on labs, DI, and referrals; overdue preventative testing to close care gaps.Manage, track, and close referrals in a timely manner and provide data and outcomes to supervisor for review.Monitor and complete follow up on the Care Gap Report.Monitor and complete follow up on the Referral Management Report.Identify referrals for Chronic Care Management and provide information to Quality Director.Identify clients for Care Management, develop Care Plans, and provide monthly follow up.Transitions of Care: Identify pediatric clients transitioning to adult PCP and complete Hospital/ED follow up to schedule PCP follow up appointment.Functional Area: Service Delivery, Partnerships, Strategic PlanningEstimated Effort: 30%Responsibilities, Deliverables, Outcomes and ExpectationsContact clients at risk of no-show or non-compliance to assist with overcoming barriers.Assess and address Social Determinants of Health issues and concerns.Assist individuals in person, via phone calls, virtually and by email providing knowledgeable and informed information and responses.Provide case management services including, at minimum, assessment, referral, and monitoring/follow-up.Maintain a courteous and professional demeanor with clients, staff, funders, community partners while promoting a positive reflection of Community Action Committee of Pike County, Valley View Health Centers and the Community Empowerment Department.Demonstrate an understanding of systems of care, entitlements and benefits, and cultural awareness.Timely completion of client notes, forms, and data entry.Maintain required records and handle confidential information in a professional manner, including HIPAA compliance.Advocates for clients to ensure equal access to services.Assists the Community Empowerment Director and Community Health Worker with special projects.Attends meetings, conferences, and training related to the duties of this position, and disseminate information to appropriate individuals.Functional Area: Agency Service and Other ResponsibilitiesEstimated Effort: 10%Responsibilities, Deliverables, Outcomes and ExpectationsCAC Committee assignments, and other agency service duties as assigned.Liaison between Community Empowerment Department staff, CAC programs/Community Partners and Providers, and ClientsOther tasks and duties as assigned by supervisor.Minimum Requirements…Education: High School Diploma or equivalentJob-Related Experience: Valid Ohio Driver’s License, clean driving record, insurable under agency policy.Proficiency in Microsoft Office.One (1) year experience providing case management services.Eligibility to obtain a Community Health Worker Certification.People Management Experience: Friendly, professional and courteous demeanor.Strong communication skills; ability to interact productively with senior staff, CAC department staff, community partners, and clients.Licenses, Certifications, and Accreditations: LPN, Vocational Nurse, Medical Assistant, or Certified Nursing Assistant; CPRPreferred Qualifications…Preference given to individuals with 1 year of case/care management.Working Conditions…Position is primarily located in a well-lighted office like environment. The employee is required to regularly sit, handle or feel objects/tools/controls, reach with hands and arms, and talk and hear. The employee is occasionally required to stand, walk, stoop, kneel, crouch, or crawl. The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. This position may require occasional evening and weekend travel. Work is fast paced and involves multiple, sometimes competing priorities, and the management of multiple tasks, responsibilities, and details.
