Job Description : Nuvance Health has a network of convenient hospital and outpatient locations — Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York — plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices. Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.

Summary:
The Birth Certificate Registrar is responsible for obtaining the necessary information for completion and filing of birth certificates with local and state departments of Vital Statistics.

Responsibilities:
1.Identify all newborns (from 3M report) and transfer name into the HIM Vital Statistics logbook and maintain same.

2.Receive and review work booklet completed by mother/physician, determine if follow-up interview/phone call is required to finalize it. Access patient record to obtain additional required data to finalize.

3.Collaborate with nursing and coding staff to validate data elements and diagnostic information, as needed; review chart to resolve discrepancies and/or to obtain missing data.

4.Enter data into SPDS computer database to generate a paper birth certificate then print.

5.Distribute printed certificate to appropriate obstetrician to sign.

6.Mail signed certificate to local Registrar within 5 days of birth.

7.File Electronic Birth Certificate worksheet in date order by month for future verification purposes.

8.Log into SPDS and electronically transmit birth certificates to NYSDOH daily

9.Handle authorized and assigned error corrections by contacting NYSDOH Bureau of Vital Statistics representative and request that they unlock the appropriate file to correct identified error.

10.Access SPDS computer database using unlock number issued by representative and make necessary correction. Print corrected birth certificate, obtain signature and resubmit to local registrar office.

11.Verify accuracy and completion of paternity affidavit and attach to completed birth certificate and submit to local registrar.

12.Accurately completes fetal death certificate as necessary to comply with NYSDOH regulations by accessing the patient record. Distribute for physician signature.

13.Forward original copy to Bureau of Vital Statistics – NYS DOH and carbon copy to local registrar within 72 hours of the time of death.

14.Attend required educational sessions and seminars to remain abreast of process and regulatory

changes.

15.Assist in other HIM related Function which may include but not limited to: Release of Information, Chart Deficiency Analysis, Chart Preparation and Scanning, and Quality/Validation of Scanned Documents .

Other information:
Required: High School graduate with a minimum of 2 years work related experience. Associate’s Degree preferred. Proficient in Microsoft Office including Word, Excel, and Outlook.

Proficient Internet Navigation.

Strong attention to detail.

Fax / Copy machine.

Organized and has the ability to prioritize requests.
