CHS is also a pivotal partner in New York City’s criminal justice reform efforts.Job Description The NYC jail system is among the largest in the nation. The Division of Correctional Health Services (CHS) of the New York City Health and Hospitals Corporation (HHC) is responsible for the delivery of medical care, mental health care and discharge planning in the jail system. CHS is a national leader in the delivery of healthcare to the criminal-justice involved population and strives toward the highest standards of care, with the goal of meeting or exceeding what is available in the community. The new Department of Geriatrics and Complex Care Services is charged with providing clinical care, care coordination and transition/reentry planning services to the sickest and most medically vulnerable people in the jail system. The program’s reentry staff will work with clients to create comprehensive discharge plans that meet their goals and care needs.Purpose of Position: Under direction of the Director of Geriatrics and Complex Care, the program manager and the program coordinator, transitional care coordinators will meet with patients to create and execute individualized discharge plans.Responsibilities Include: Complete intake assessments and transition plans for patients who meet eligibility criteriaEngage patients around completing a health care proxy and identifying community resources and sources of social supportCreate individualized discharge plans for each patient according to the patient’s goals and needs and take responsibility for ensuring tasks are completed. Tasks may include coordinating medical appointments, ensuring medications are called into a pharmacy or confirming transportation between jail and community dwelling is arranged.Participate in patient case conferences with other discharge planning program staff. Document intake assessment, transition plan and collateral information in the electronic health record.Additional tasks and projects as requested by Correctional Health Services.Minimum Qualifications 1. A four-year high school diploma or its educational equivalent and five (5) years of satisfactory full-time experience providing direct counseling, guidance, crisis-intervention or informational and referral services to inmates of a correctional facility, substance abusers or a similar client population; or2. Education and experience equivalent to “1” above. Thirty (30) semester credits from an accredited college will be considered equivalent to one (1) year of the required experience.
