Job Description : Our Assisted Living Facility has an opening for a Case Manager, Sunday – Thursday. The Case Manager is responsible for coordinating the Community’s case management system that maintains the continued awareness of each resident’s well-being allowing each resident to attain the highest practicable level of physical, mental and social well-being.Please note: this position is not a 9-5 position, it requires flexibility in working nights and weekends, and has great potential for growth opportunity.QUALIFICATIONS:Education:A bachelors or masters degree from an accredited college or university with major work in human resources or service delivery, and one year of full-time experience in the provision of services to a dependent adult population; or an associates degree from an accredited college or university with major work in human resources or service delivery, and three years of full-time experience in the provision of services to a dependent adult population.Experience:Two years of experience in a related field.Job Type: Full-timePay: $75,000.00 per yearBenefits:401(k)401(k) matchingDental insuranceHealth insurancePaid time offVision insuranceSchedule:8 hour shiftMonday to FridayOn callWeekend availabilityCOVID-19 considerations:As per the Department of Health regulations this job requires Covid-19 Vaccination.Work Location: One location
