Job Description : We are a non-profit organization serving brain injury survivors and adults with disabilities. Our mission is to help guide them from surviving a terrible tragedy or illness to rebuilding their lives and maximizing their independence. Our services help young adults move from nursing home environments to living more full and happy lives in their own homes and community.We are currently seeking a highly focused, organized, and flexible candidate for an open Service Coordinator/Care Manager position with our organization.Service Coordination services coordinate all the community-based services individuals with disabilities and chronic medical conditions need to support them to live as independently as possible. This can range from securing financial benefits, finding and securing a place they can afford to live, arranging medical services, vocational/educational services transportation, therapies and NYS Department of Health Medicaid Waiver services. The Service Coordinator monitors the participant’s services and makes adjustments as needed or requested.We are specifically seeking a candidate who is able to work remotely from home, however, monthly/as needed home visits with consumers and community providers are required. Travel will vary based on caseload but could include Monroe, Ontario, Wayne, Yates, Seneca and Livingston counties.Join our Team and you will enjoy meaningful work, appreciation and recognition and positive working relationships!QualificationsEducation: Bachelor’s Degree in Social Work, Psychology or another related fieldExperience: 3 years of experience providing case management/service coordination with people with disabilities and/or seniors. Experience MUST include linkage and referrals to community resources for people with disabilities (ie..physical/health disabilities, brain injury or developmentally disabled)Skills: Candidates must demonstrate excellent organizational skills with attention to detail, strong verbal, written and computer skills and the ability to manage multiple projects, self-prioritize and adjust quickly to shifting priorities.
